Many of our footwear products provide the necessary assistance impossible with a standard retail shoe but have a similar appearance to the retail products available through retails outlets. Doctors and Referrers are asked to complete a Referral Form when recommending a patient seek our services.We value team work and appreciate your time to fill in a referral. Please state your objective (eg support unstable foot and ankle) and notify us about potential risk factors ( eg Diabetic foot with ulcer history). If you want us to do something specific please specify what you would like us to do ( eg Heel only rise of 1 cm on left shoe to reduce pull on tendon Achilles). If you are specific, we will do as you request if your referral is broader we will do our own assessment. Referral Forms may be the supplied by the Doctor/Referrer or the ShoeTech Referral forms may be used. Get prompts from referral form i.e. for Prescription Pad.pdf
